Children's Mental Health Case Manager

Job Description:

Program Summary: Children’s Mental Health Case Management (CMHCM) provides in-home support and guidance for residents of Hennepin County with children up to age 18 (or 21 if in high school) who experience a mental health diagnosis which significantly effects their life.

Position Overview: St. David’s Center offers a unique opportunity to provide support and case management services to children and their families experiencing mental health challenges. We are currently seeking a full-time, Bachelor’s level candidate to join our Children’s Mental Health Targeted Case Management Team.

Responsibilities include:
  • Assisting clients in gaining access to needed medical, social, educational, and other services to address mental health needs
  • Coordinating services designed to enable a child to improve or maintain functioning in order to remain with his/her family in the community
  • Developing supportive relationships with the child, the child’s family, and the team serving the child, while consistently maintaining professional boundaries and reflective practice
  • Collaborating with families to plan services while focusing on the strengths of the child and of the family, viewing the child’s needs holistically, and recommending services that will help the child reach or maintain his/her optimal level of functioning in a community-based setting
  • Meeting in the family home or community with the child/youth and their caregiver a minimum of once a month

Schedule: 40 hours per week, M-F. This position requires working two evenings per week.

Pay: DOQ. This position is benefits eligible, including qualifying for loan forgiveness!

  • Bachelor’s degree in Behavioral Sciences or a related field from an accredited college or university
  • A minimum of 2,000 hours of supervised experience in the delivery of mental health services to children with mental health concerns
  • Must have a valid driver's license, reliable transportation, and an acceptable driving record
  • Experience with families from diverse communities
  • Strong skills in assessing and identifying the needs of children with mental health diagnoses
  • Basic knowledge of the DSM 5 diagnosis
  • Ability to work as an interdisciplinary team member
  • Strong knowledge of community resources
  • Excellent written and oral communication skills
  • Strong organizational, time management and attention to detail skills
  • Experience with Word and Excel
  • Prior mental health targeted case management experience
  • Experience with electronic medical records
